Nighttime should offer rest, but spasticity can make settling into bed, staying asleep and changing position unexpectedly difficult. What does nighttime spasticity feel like?

Spasticity is a velocity-dependent increase in resistance associated with injury to upper motor neuron pathways. In everyday life, however, people often use the word for several experiences: a leg that feels rigid, an arm that draws toward the body, repeated ankle beats, sudden whole-limb movements, or pain caused by a muscle being held in a shortened position.

A spasm is an involuntary burst of muscle activity. A flexor spasm may pull the hip and knee toward the body; an extensor spasm may straighten the leg and point the foot. Clonus is a rhythmic series of contractions, often triggered when a joint is moved or placed in a particular position. These phenomena can accompany spasticity, but they are not interchangeable with the stretch response measured during a clinical examination.

Other conditions can imitate part of the picture. Night cramps are usually painful, localized contractions. Restless legs syndrome produces an urge to move, often with uncomfortable sensations that improve temporarily with movement. Periodic limb movements occur during sleep and may not be noticed by the person. Dystonia, neuropathic pain, arthritis and a fixed contracture can also disturb sleep. Why can spasticity and spasms feel worse at night?

In bed, a hip, knee, ankle, shoulder or elbow may remain in one position for a long time. Bedclothes, a poorly placed pillow, an unsupportive mattress edge or pressure from a brace can add sensory input or mechanical strain.

A painful shoulder, hip, back or joint can increase protective muscle activity, while interrupted sleep lowers resilience the following day. A marked or sudden increase in spasticity may be the nervous system’s response to another problem. Common triggers include urinary infection or retention, constipation, skin irritation, pressure injury, an ingrown toenail, pain, fever, dehydration, tight clothing, a poorly fitting orthosis and an uncomfortable seating or sleeping position. Sometimes the trigger is not painful in the usual way, particularly when sensation is reduced.

How poor sleep affects the next day

Repeated awakenings can cause daytime sleepiness, slower thinking, irritability and lower tolerance for pain. Transfers, walking and self-care may require more effort. A caregiver may also lose sleep while helping with repositioning, hygiene or bedding, which can affect safety and wellbeing for both people.

Not every sleep problem in a person with spasticity is caused by muscle tone. Snoring with pauses in breathing, waking gasping, morning headache and pronounced daytime sleepiness may suggest sleep apnea. An irresistible urge to move the legs may point toward restless legs syndrome. Anxiety, depression, nocturia, reflux and medication effects are other possibilities. Practical nighttime strategies

Position for comfort and skin protection

There is no single correct sleeping position for everyone. Pillows or other supports may help keep a limb comfortable, reduce friction and avoid a prolonged end-range posture. The arrangement should not create pressure behind the knee, against the heel or over a bony area. People with reduced sensation, fragile skin, breathing problems or complex positioning needs should ask an occupational or physical therapist for an individualized plan.

Reposition slowly. A rapid stretch can provoke a stronger response in a spastic muscle. If a limb begins to spasm, forcing it abruptly against the movement may increase discomfort or injure the person or caregiver. Pause, support the limb, check for pain or an obvious trigger, and use the techniques already taught by the clinical team.

Follow the agreed movement plan

Gentle range-of-motion work or a familiar stretching routine may be useful when it is part of an established rehabilitation program. More force is not necessarily better. Forceful stretching through pain, aggressive pulling during a spasm or unplanned overnight splinting can damage skin and soft tissue. Orthoses should be checked for fit, pressure marks and changes in swelling.

Support sleep, not only muscle tone

A regular sleep and wake time, a dark and comfortable room, reduced late-evening caffeine and a wind-down routine can support sleep even when they do not change spasticity itself. Plan toileting and essential care in a way that reduces unnecessary disruption. If nighttime transfers are difficult, ask for a review of equipment and caregiver technique rather than improvising a risky lift.

How clinicians approach treatment

Current clinical guidance emphasizes individualized, goal-directed and multimodal care. The first question is what needs to improve. A useful goal might be reducing awakenings from five to two, allowing comfortable side-lying for three hours, easing painful calf spasms, protecting the palm or making overnight care safer. A tone score alone cannot show whether sleep has improved.

The clinician first checks for modifiable triggers and distinguishes focal from generalized symptoms. Positioning, seating, movement practice, pressure care and caregiver education may be part of the plan. For a localized pattern, botulinum toxin treatment may be considered when it matches a clear goal. More widespread symptoms may lead to discussion of oral antispasticity medicines. Severe, persistent generalized spasticity may prompt specialist evaluation for options such as an intrathecal baclofen pump.

Medication timing can be relevant, but it should be reviewed with the prescriber.
